ECM Lead Care Manager

The ECM Lead Care Manager serves as the primary point of contact for Managed Care Plan members, coordinating whole-person care management for the Birth Equity population, including pregnant and postpartum individuals and families with infants up to 12 months old. This field-based role emphasizes engagement, assessment, care planning, resource linkage, and collaboration with multidisciplinary teams to address medical, behavioral, and social determinants of health. The position involves in-person, community-based, virtual, and telephonic engagement with up to 40% local travel required.
