Facilitator

The Facilitator ensures the High-Fidelity Wraparound process is implemented with fidelity, engaging families and youth to develop individualized care plans based on their strengths and needs. This role involves leading Child and Family Team meetings, coordinating crisis and safety plans, and providing direct services to support children and youth with intensive mental health challenges. The position requires maintaining a strengths-based, culturally responsive approach while navigating complex public service systems to promote family-driven success and stability.
