Family Support Specialist

This position provides referrals and supportive services to adoptive families and permanent guardians in Circuit 12, utilizing a wrap-around approach to stabilize youth and coordinate behavioral health and community services. The role involves tracking family progress, facilitating team meetings, and assisting with crisis planning and higher levels of care. It may require being on call during evenings and weekends on a rotating schedule.
