Field Care Coordinator

Care Coach 1 at Humana conducts in-person assessments of Medicaid members in homes, assisted living and long-term care settings to evaluate needs and develop member-specific care plans. The role focuses on connecting members and families with appropriate community and clinical resources to promote wellbeing and maintain members in the least restrictive setting. This position is based in the Clearwater, FL area and involves significant local travel to meet members where they receive care.
