The Assessment Nurse conducts field and virtual assessments for prospective and current members across downstate New York to determine enrollment, service needs, and continued eligibility under Department of Health requirements. The role completes required documentation and coordinates with the Care Management Team to support Healthfirst’s long-term care plans such as CompleteCare and Senior Health Partners.
