Forensic Case Manager

The Forensic Case Manager provides case management to incarcerated individuals with co-occurring disorders transitioning to the community from the Berks County Jail System. The role involves assessing treatment and social service needs, developing individualized strength-based service plans, and facilitating interdisciplinary approaches to support successful re-entry and recovery. This position requires collaboration with behavioral health and medical departments to identify appropriate community resources and ensure continuity of care.
