About this role

Responsibilities

• Support individuals and families accessing Primary Care services with wellness needs and crisis intervention.
• Provide culturally appropriate social work services to improve wellness and empower First Nations, Inuit, and Mi'kmaq peoples.
• Manage caseload targets and maintain direct service requirements while delivering brief counseling and assessments.
• Collaborate with the Mental Health Team on case reviews, team meetings, and Indigenous Cultural Safety training.
• Advocate for clients by completing required paperwork, screening assessments, and linking them to appropriate community resources.
