Health Coordinator - FT/Swing

The Health Coordinator supports Pioneer Human Services' mission by coordinating and monitoring client physical and behavioral health services within residential treatment facilities. This role assists in ensuring continuity of care by facilitating communication between clients, nursing staff, medical and behavioral health providers, pharmacy services, and program staff. The Health Coordinator supports the intake process, coordinates internal and external medical and behavioral health appointments, assists with medication monitoring, and gathers and communicates relevant health information to appropriate clinical staff.
