About this role
The Health Home Care Manager provides supportive Health Home care management for individuals identified as high users of services, coordinating access to medical, mental health, social and other community supports. The role is based at St. Mary’s Campus with a Monday–Friday schedule and a hybrid work opportunity. The position focuses on enrolling and engaging clients in a written plan of care to improve access and continuity of services.
