Otterbein SeniorLife is hiring a Home Health & Hospice Care Navigator to grow and drive post-acute skilled Home Health census across Bellefontaine, Marysville, Dublin and New Albany, Ohio. The role represents Otterbein to hospitals, skilled nursing facilities, senior communities and community partners to generate referrals and ensure smooth admissions to Home Health and Hospice services. This is a mission-driven position within a faith-based, not-for-profit senior care organization.
