Home Visitor

This role involves identifying and supporting overburdened families through home visits using the Healthy Families America model. The position requires conducting interviews, assessments, and maintaining accurate documentation while adhering to confidentiality and privacy standards. The Home Visitor also participates in staff development, clinical supervision, and community outreach to promote program awareness.
