The Highlands At Wyomissing
The Hospice RN Case Manager provides nursing care and case management to patients and families within the Highlands at Wyomissing campus, coordinating interdisciplinary plans of care and supervising ancillary staff. The role involves comprehensive assessments, developing and updating care plans, on-call duties, and participation in quality improvement and continuing education activities.
The Highlands at Wyomissing® is a non-profit Life Care retirement community located on over 113 acres in Wyomissing, Pennsylvania, offering independent living, assisted living, memory support, and short-term rehabilitation services.
