Croí Health
The Hospice Social Worker provides counseling and targeted interventions to homebound patients and their caregivers receiving palliative or hospice services, conducting psychosocial assessments and contributing to care plans. This full-time role works within a home-based interdisciplinary team to improve patient outcomes and support self-management of biopsychosocial challenges.
