Housing Case Manager

The Housing Case Manager provides housing-focused case management to participants experiencing homelessness or chronic homelessness, including high-cost health users and Medi-Cal beneficiaries. This role is responsible for rapid engagement, consistent documentation in HMIS, landlord and housing navigation work, and housing retention support aligned to HHC contract requirements. The position involves engaging households, completing housing applications, and conducting home visits to verify safe and suitable housing conditions. The role also requires coordinating with landlords, attending case meetings, and providing weekly status updates to management.
