The Housing Medical Case Manager I serves as part of the UofL Ryan White Kentucky Care Coordinator Program case management team, providing housing-focused case management and advocacy services to Ryan White-eligible clients. This role supports clients in obtaining and maintaining stable housing while coordinating comprehensive, client-centered care to promote treatment adherence, self-sufficiency, and improved health outcomes. The position ensures services are delivered in compliance with Ryan White Program requirements and University of Louisville policies.

Responsibilities

Provide housing-focused case management and advocacy services to Ryan White-eligible clients
Conduct comprehensive eligibility, acuity, and housing assessments including VI-SPDAT
Coordinate housing transitions and monitor client progress to ensure successful implementation
