Housing Support Worker

Family Transition Place is seeking a Housing Support Worker to provide Housing First focused support, increasing housing stability and preventing eviction for vulnerable individuals in Dufferin County. The role involves delivering comprehensive, integrated care services through a multidisciplinary team, engaging with landlords, identifying housing opportunities, and supporting clients to secure and maintain safe housing. This position requires collaboration with internal and external partners to enhance the quality of life and overall well-being of clients experiencing homelessness, mental health challenges, and substance use challenges.
