IHART Connector

The IHART Connector supports unhoused residents of Santa Cruz County experiencing serious mental illness or co-occurring disorders by conducting outreach, case management, and connecting them to housing and community resources. This role involves direct service delivery, collaboration with multi-agency teams, and maintaining accurate documentation to monitor client progress and treatment goals.
