Intake LPN

The Intake LPN provides knowledgeable, compassionate, and clinically sound nursing care through initial and ongoing patient assessments and care coordination for Residential, PHP/IOP, and/or LIR clients. This role is essential to ensuring a safe, thorough, and supportive admission process while maintaining accurate clinical documentation and collaboration across treatment teams. The Intake Nurse supports patients and families during a potentially stressful transition into care and ensures all medical, safety, and compliance requirements are met upon admission.
