Intake Worker

The Intake Worker serves as the first point of contact for individuals accessing the Connected Care centre in Southern Tasmania, supporting intake, screening, and assessment processes to identify appropriate levels of care. This role involves using the Initial Assessment and Referral Decision Support Tool to understand participant needs and facilitate timely access to services. The position focuses on promoting informed choice, autonomy, and self-determination while collaborating with a multidisciplinary clinical team to deliver person-centred mental health care.
