Licensed Clinical Social Worker

The Licensed Clinical Social Worker provides clinical social work and complex discharge planning to patients and families with psychosocial needs, including crisis intervention and mental health assessments. This role collaborates with an interdisciplinary team to develop multidisciplinary discharge plans for high-risk populations and ensures psychosocial issues are addressed across the continuum of care. The position also involves supervising social work staff and students while advocating for patient empowerment and independence.
