MDS Coordinator (RN)

The MDS Coordinator (RN) ensures the timely completion of accurate assessments and interdisciplinary care plans that comply with Federal and State guidelines. This role involves identifying resident acuity and needs to determine specific care requirements and communicating these expectations to families and responsible parties. The position is responsible for completing all assessments, Minimum Data Set (MDS) documentation, and care plans for assigned residents.
