The Home Health Medical Social Worker provides in-home psychosocial support and care coordination for patients across Camden, Burlington, and Gloucester counties. The role supports transitions from hospital to home and promotes patient independence, safety, and well-being as part of a multidisciplinary team. This is a full-time, pay-per-point position with training and 24/7 clinical support from Jefferson Health at Home by BAYADA.
