Mental Health Professional/Case Manager

Olympic Health & Recovery Services is seeking a Mental Health Professional/Case Manager to join the Homeless Outreach Stabilization and Transition (HOST) team, providing support and case management to individuals experiencing homelessness and severe behavioral health disorders. The role involves engaging vulnerable populations in non-traditional settings, developing client-driven treatment plans, and coordinating access to community resources and housing. This position operates within a multidisciplinary team following the Assertive Community Treatment model to assist clients in achieving stability and long-term recovery.
