Full-time Nurse Practitioner role providing primary care to frail older adults enrolled in the Neighborhood PACE program. The NP will serve as an independent primary care provider for a panel of participants across geriatric settings including home care, nursing homes, and a day center, working within an interdisciplinary care team. Strong preference for candidates with significant experience caring for frail adults; Adult or Family Medicine certification considered. Competitive salary with excellent benefits.
