About this role
A Nurse Practitioner at PHP delivers high-quality, person-centered primary care to seniors and other residents across skilled nursing, assisted living, and independent living settings, and via telehealth. The position is based at Lake Pointe Healthcare Center in Lorain, OH and is scheduled first shift, Monday–Friday with no on-call, weekend, or holiday coverage. The NP collaborates with interdisciplinary teams and primary care providers to manage an assigned population and improve outcomes.
