About this role
A Nurse Practitioner at Strive Health provides advanced nursing care for adults with chronic kidney disease (CKD) and end-stage kidney disease (ESKD) as part of an interdisciplinary clinical team. The role focuses on delivering high-quality, patient-centered care across settings (home, clinic, partner sites, and telehealth) and acting as an advocate, navigator, and trusted clinician for patients. This position reports to the Senior Manager, Nurse Practitioner and supports Strive’s mission to improve kidney care outcomes.
