Occupational Therapist

This role involves providing comprehensive occupational therapy services to low-income seniors within a Program of All-Inclusive Care for the Elderly (PACE) model. The therapist will collaborate with an interdisciplinary team to assess participants, develop care plans, and deliver interventions that support aging safely and with dignity in their communities. The position requires conducting assessments, maintaining documentation, and educating participants and caregivers on functional daily activities.
