Occupational Therapist

This role involves supporting safe, timely, and person-centred discharges from hospital as part of the Hospital Discharge team. The Occupational Therapist will collaborate with health colleagues, families, and partners to help individuals transition to the most appropriate settings while promoting recovery and independence. The position is based in West Lancashire and contributes to the council's vision for Adult Social Care.
