Social Worker

This role involves supporting safe, timely, and person-centred discharges from hospital by collaborating with health colleagues, families, and wider partners. The Social Worker helps individuals transition to the most appropriate settings, promoting recovery, independence, and positive discharge experiences. The position is part of the Hospital Discharge team within the Adult Social Care service in West Lancashire.
