Moderate Care Coordinator

The Moderate Care Coordinator provides timely, holistic care coordination to children enrolled in OhioRISE, utilizing a wraparound-informed model to manage behavioral, medical, and pharmacy health services. This full-time role involves collaborating with Child and Family Teams to develop and monitor individualized care plans while maintaining a caseload of no more than twenty-five members. The position requires conducting comprehensive assessments, facilitating service planning, and ensuring compliance with federal waiver requirements.
