OhioRISE Moderate Care Coordinator

The OhioRISE Moderate Care Coordinator role involves delivering community-based, wraparound care coordination for children with complex behavioral health and multi-system needs across Champaign and Logan Counties. This position focuses on guiding families through the OhioRise program, building stronger support systems, and ensuring effective collaboration with providers and community partners.
