Oncology Navigator - Full time

An Oncology Navigator guides patients through the cancer care continuum, providing education, advocacy, and coordination of services from diagnosis to survivorship. This role acts as a liaison between patients and the multidisciplinary care team to eliminate barriers to access quality medical and psychosocial support. Duties include managing referrals, utilizing distress screening tools, conducting presentations, and documenting care in electronic medical systems.
