About this role

The Outreach Specialist will educate community partners, prospective participants, families, and caregivers about Habitat Health’s PACE program and build relationships to drive awareness and referrals. The role also supports and manages the enrollment process for prospective participants to ensure a smooth transition from outreach to enrollment.
