Participant Enrollment Specialist

The Participant Enrollment Specialist guides seniors and their families through the initial steps of joining the program by building relationships, completing assessments, and coordinating documentation. This role serves as the main point of contact for referrals, ensuring a smooth and supportive enrollment experience while collaborating with the Interdisciplinary Team and community partners. The position focuses on maintaining accurate records, tracking enrollment progress, and supporting participants through the intake process.
