Patient Assessment Coordinator - FT Days

The Patient Assessment Coordinator role involves coordinating the process of providing accurate and timely data for Medicare patients' Prospective Payment System and quality reporting programs. This position oversees data collection on the IRF/PAI form, ensures supporting documentation is present, and provides staff education to ensure compliance with CMS guidelines. The Coordinator reviews each IRF/PAI form completely, accurately, and timely, monitors the accuracy in scoring functional status, and inputs appropriate information into the eRehabData system. The role is also responsible for coordinating the collection of patient information from medical records and completing assigned sections of the IRF/PAI form.
