The Patient Engagement Specialist I supports Oak Street Health’s Patient Engagement and Population Health team by conducting outreach to engage patients in the clinic’s care model. The role focuses on connecting with patients via home visits and telephone to help them access appropriate primary care services and navigate available wrap‑around supports. This position works regionally and partners with clinical and community teams to improve patient engagement and care access.
