About this role

Responsibilities

• Facilitate patient access by managing care pathways and appointment scheduling
• Serve as first point of contact for patients and community referrers
• Coordinate communication between patients and multidisciplinary clinical teams
• Prepare data for weekly Multi-Disciplinary Meetings to support treatment planning
• Identify and prioritize urgent patients for registration and initial appointments
