The Recovery Navigator will deliver the IMPROVE-65 patient-centred recovery program for older adults following hospitalisation, supporting improved care and outcomes. The role coordinates care between acute hospitals and general practitioners, conducts telehealth consultations, and assists patients with recovery planning and goal setting. This is a fixed-term, part-time position based at Cabrini Malvern within the research program.
