Patient Navigator

Sun River Health is seeking a full-time Patient Navigator to support its Bedford Avenue site in Brooklyn, NY. The role involves identifying individuals in need of preventive and primary health care services and linking them to necessary resources, including acute, chronic, and dental care, as well as prenatal and pediatric services. The position also includes training and mentoring Patient Representatives and facilitating patient appointments across various departments and outside agencies.
