Patient Navigator

Sun River Health is seeking a full-time Patient Navigator to support its Peekskill, NY site by identifying individuals in need of preventive and primary health care services and linking them to necessary resources. The role involves facilitating patient appointments, assisting with referrals to specialty care and public health insurance programs, and providing ancillary support such as translation and transportation assistance. The position also includes training and mentoring Patient Representatives while tracking patient progress in the electronic health record system.
