Patient Navigator

Sun River Health is seeking a Full-time Patient Navigator to support its Greenport, NY site. The role involves identifying individuals in need of preventive and primary health care services and linking them to necessary resources, including care for acute, chronic, and communicable diseases, dental services, and pediatric care. The position also includes training and mentoring Patient Representatives and facilitating patient appointments and referrals within the organization and outside agencies.
