Patient Navigator

Sun River Health is seeking a Full-time Patient Navigator to support its New Paltz, NY site. The role involves identifying individuals in need of preventive and primary health care services and linking them to necessary resources, including acute, chronic, and dental care. The position also includes training and mentoring Patient Representatives while facilitating patient appointments and overcoming barriers to care.
