Peer Mentor

The Osborne Association seeks a full-time Peer Mentor to provide targeted mentorship to older adults transitioning from incarceration, utilizing the Critical Time Intervention model to foster reentry stability. This role involves guiding participants through structured phases to secure housing, employment, and benefits while building rapport to mitigate reentry anxiety. The position requires collaboration with a multidisciplinary team to execute warm handoffs to long-term community providers and ensure successful reintegration.
