Peer Navigator

The Peer Navigator supports members experiencing or at risk of homelessness in the South County Riverside, CA area by managing their health and motivating behavioral changes through education and peer support. This patient-facing role involves connecting individuals to housing and community resources, providing accompaniment to medical appointments, and collaborating with healthcare teams to coordinate care. The position requires maintaining accurate documentation in electronic health records and working under the supervision of a Social Worker to ensure proper case escalation.
