Float Peer Navigator

The Float Peer Navigator supports members experiencing or at risk of homelessness in San Diego by managing their health through education, peer support, and resource navigation. This full-time role provides continuity of care during staff leaves, maintains an assigned member panel, and assists with onboarding, eligibility activities, and cross-team operational support. The position requires travel between service areas and collaboration with clinical and community teams to improve health outcomes and housing placement.
