The Person-Centered Services Navigator provides person- and family-centered navigation, service coordination, and individualized planning support for individuals with autism, developmental disabilities, and/or complex support needs. This role involves helping families access services, build skills, and increase independence while coordinating with DDS, schools, and community resources. The position requires travel throughout Eastern Massachusetts and flexible scheduling to meet the needs of the individuals and families served.
