Physical Therapist

The West PACE Physical Therapist provides restorative and rehabilitative physical therapy services to senior participants both at the center and in their homes, collaborating with physicians, occupational therapists, and the interdisciplinary team to maximize independence and safety. The role includes conducting assessments, delivering treatment interventions, supervising therapy assistants, and ensuring comprehensive documentation and compliance with West PACE policies.
