Psychosocial Support Worker - HOPE (Melton)

The Psychosocial Support Worker joins the Melton Mental Health and Wellbeing HOPE Team to provide brief therapeutic interventions for individuals at risk of suicide. This full-time role supports clients for 12 weeks post-discharge from emergency or inpatient settings, offering assertive and coordinated care to reduce the risk of re-attempts. The position involves working closely with community mental health teams and multidisciplinary partners to deliver safe, high-standard care across the Melton, Hume, and Brimbank catchment areas.
