Recovery House Transitional Case Manager

The Recovery House Transitional Case Manager provides case management and crisis intervention services to residents using a recovery-oriented, trauma-informed, and person-centered approach. This role supports formerly homeless individuals in maintaining stable housing, strengthening recovery, and achieving greater independence while fostering a positive residential community. The position collaborates with residents, service providers, and community partners to coordinate access to mental health, substance use, medical, vocational, and other supportive services that promote housing stability and successful community integration.
