Montgomery County Coalition for the Homeless
The Case Manager works with formerly homeless, disabled single individuals in shared houses using a housing first, trauma informed model of service delivery. The role provides an overall support system for clients, referring them to local agencies for vocational, educational, and health services. The position involves maintaining an active caseload, conducting house meetings, and monitoring client progress in substance use recovery and mental health status.
